Need to see a doctor right away? All you have to do is call our scheduling professionals at 219-736-8800 for our Merrillville and St. John offices or 219-836-2201 for  our Munster office.  We make every attempt to meet your scheduling needs as soon as possible. 

Please be sure to bring any recent test results or imaging results that you may have recently had that would help us evaluate and treat you better.  Please also bring your insurance card, a picture ID and a list of any medications you may be taking.  And for your convenience you may visit our download patient forms tab so that you may download our patient registration forms, our patient information sheets and our HIPAA forms so that you may easily complete the forms and bring with you to your first visit.   You can also find directions to our office by clicking on the location tab at the left of this page.
